The configuration of your EFI folder. That's the issue.
 
Is anyone els having problems with thunderbolt three hot plug not working after sleep ?
 
Here is my information just in case it helps for future troubleshooting.

I have followed this tutorial and I have found that hot plug works perfectly
I am usbinjectall and have a custom SSDT-UIAC.aml and have made sure to include my thunderbolt 3 port
Sleep and wake from sleep is working perfectly
When I wake from sleep thunderbolt 3 no longer detects a device
I have tried leaving a Device plugged into the thunderbolt 3 port, and even when a device is left into the thunderbolt 3 port and is never removed the thunderbolt 3 port stops detecting devices.

I believe it is a sleep problem and not a problem with ThunderboltReset.kext but I wanted to post it here just in case someone els was having this issue.

UPDATE - Thunderbolt 3 will work after sleep for me, but not with usb 3.0 devices

-Thunderbolt ethernet (working after sleep and hot-plug)
-Thunderbolt to Hdmi (working after sleep and hot-plug)
-Thunderbolt to Display port (working after sleep and hot-plug)
-Thunderbolt to USB 2.0 (working after sleep and hot-plug)
-Thunderbolt to USB 2.0 extender with a usb 3.0 Device (working after sleep and hot-plug)

Thunderbolt to USB 3.0 (working with hot plug) (not working after sleep)
Thunderbolt to thunderbolt (not working after sleep)

Have not tested a EGPU because I do not have one to test.

I hope this testing has helped. So the hot plugging seams to work after sleep, just some things do not

Update
- So I believe that I completely fixed it, I changed some bios settings
What sesames to have fixed it for me was changeing adaptor config to no security and auto switch to bios assist enumeration

I hope this helps someone els
