Upgrade from Mavericks failed

I got a new SSD in my laptop, together with a hybrid drive, and restored Mavericks to the SSD. After that I managed to change the boot manager from Chameleon to Clover. Several attempts to create a working High Sierra or Sierra installer failed, there is always a kernel panic, apparently caused by ApplePowermanagement. I used the same EFI as for Mavericks, adapted it according to suggestions I found in forums, and created it new, all in vain. Does anyone have any suggestions?
My Lenovo B570 with i3-2330M and HD 3000 is quite old, but with the new SSD and a later OS it could still be usable for years. By the way, clover does not recognize Windows and Ubuntu on the other drive.

Are you going to use CLOVER or OC?
 
Clover, because OC failed, despite of studying the instructions for hours and following them in every detail.

Can you provide the error you get when booting the OC? A picture/video would be nice.

Also, provide the OC EFI to check.
 
I attach a screenshot of my EFI folder and some pictures extracted from a video. The screen is a mirror, so I censored some pictures. The one I called boot fail shows a new effect, never experienced before. And it is all Clover, OC is deleted due to being useless on this laptop.

This looks more of a Still waiting for root device error. This happens when the installer path is no longer accessible. That means the port you boot from goes off during boot or the storage controller. When the EFI is in the ESP and this happens, it cannot detect the storage controller.

Its really not an issue if you would like to use CLOVER for your own good reasons. Having said that, provide the Laptop BIOS Settings and attach a copy of your current CLOVER EFI as well.

Also, what's the Clover version you're using?

The Clover version is 4045, after finding an old installer app from a different site. I already uploaded a screenshot of my EFI folder.
Clover on the SSD that boots Mavericks successfully is version 5122. First I tried the latest version that didn't work at all. In previous attempts I had copied Clover from the SSD to the installer, but apparently the settings are incompatible with higher versions of macOS.

Its not as per the guide. But here are a few things:

If you want to use CLOVER, then first map the USB ports. The kexts you're using are depreciated now. So, basically, you gotta update CLOVER+Kexts, map ports, adjust to the new guide and you should be able to perform an upgrade.
