I think this is easier to reply to.
So my system specs:
HTML:
Z77X-UD5H-F16_Modded_Bios (f16 mod 11)
i7-3770k - HD4000 (Pure compute)
32GB DDR3-1600
AMD Radeon Pro WX 7100 (Vendor HP) [PCIe Slot 1]

1x 250 GB SSD for MacOS
1x 250 GB SSD for Audio
1x NVMe SSD via Adapter for audio [PCIe Slot 3]

PCIe Slot 2: Avid HDX [Professional Soundcard]

PCIe X1 Slot 1: FireWire 800 Card
PCIe X1 Slot 3: Apple Airport Card
SMBIOS: iMac 13,2

So I swapped out the NVMe Drive on PCIe Slot 3 for this test.
Had flashed: NVM 43 Mod, NVM 50 Mod, NVM 33 Z390 Mod

But non of these shows the card in the system.
Non of the native Apple Thunderbolt drivers are being loaded, I'm guessing it has to do with this ?

My system doesn't have Thunderbolt settings in the BIOS, it's like a real Mac Pro 5,1.
In the first guide link you're talking about connecting an Thunderbolt Device before booting.
But when I do this, I don't see it in the 'IOREG'
So I can't make an SSDT for it.

Firstly, you'll need to flash a Official BIOS.
If the TB Device is detected and working, it will show up in IOReg.

Yes, firstly you should flash a Official BIOS and then check your work. Once the Hotplug is setup, you can try to flash the modded BIOS and then check.

So what I tried an hour ago was putting the card in the second PCIe slot (labeled PCIEX8) instead of the 3th (PCIEX4)
And bam, card is recognized.

But when looking at IOReg for the SSDT it's seems strange:

So, what do I need to do now ?
I'm feeling that my IOReg is completely wrong...

Nothing seems strange. The IOReg is showing the TB Card. Also, it looks like you have flashed the TB Card.
So, what do I need to do now ?
I'm feeling that my IOReg is completely wrong...
It's working as expected. The Device is being shown in System Report and the IOReg has the same information. The device should be working.

Yes, correct, it's working and yes, I'm using a flashed firmware.
I don't have an SSDT yet for HotPlug, that's why I'm asking how to make one, because in other screenshots is see something like RP05 and RP21 and here I got something totally different

That's very normal. It actually depends on the slot. The slot may seem as RP05, RP21, BR1A and BR3A (X99/X299/X299X), PEG0, PEG1. These are all the device address. With a flashed card, the Hotplug should normally works. Test if it works, if not, the guide will be updated accordingly.
