Kaby Lake Hackintosh - macOS boots to the log on page fine, but screen quickly goes black

You seem to have just copied the frame buffer data without understanding the guide. As per your system specs, your system has 1xHDMI and 1xVGA which means two connectors only, the rest are not relevant. In addition, there is no DP on your system but you're patching a connector for DP.
I understood the guide. I thought VGA would be something, but yeah, I only have one actual HDMI port and one VGA. Should I just have con0 and con1 then? con0 VGA, con1 HDMI?

It really depends on what the physical connector responds to. For example, on a system, with a single HDMI port, it can be con1 or even con0. On other hand, it can be con2. So, once you determine, you can patch that specific connector. It's already explained in the guide. Read it carefully.
 
UPDATE:

I decided to try hibernationfixup and put the computer to sleep. Upon resume macOS freezes entirely. (And the first monitor doesn’t turn on despite forcing all displays to online) image.jpg

Hibernation isn't that well supported on non-Apple computers and is highly discouraged. Just normal Sleep/Wake should be fine.

As you haven't provided the model number, so its not clear which monitor is using what connector and if your system has just 1xVGA and 1xHDMI port, how are you connecting both the monitors?

The Samsung ViewMaster monitor (primary) uses a DVI-D port for connection. While the Smart TV uses HDMI port

For viewmaster, I connect my adapter (DVI to HDMI) to the only HDMI port on my system while my Smart TV, I used HDMI to HDMI but used a USB HDMI adapter (which works)

So, one is HDMI to HDMI and the other monitor is DVI to HDMI via the USB HDMI Adapter? Is that correct?

wait! The USB HDMI adapter is using HDMI to HDMI (so it’s connected with the USB adapter with USB port), while the DVI to HDMI is connected to the HDMI port on my computer. Sorry!

Well my efforts is now shifted to fixing the sleep mode in macOS. I did the force display online thing which now makes my DVI monitor show on startup. Yay! But as for the sleep mode, when macOS resumes from sleep, the OS locks up. If I’m lucky only the second monitor (Smart TV) will show a picture and work (sometimes it’ll show a picture but macOS locks up), but when I unplug the DVI monitor port and plug it again the OS will lock up. Also I removed hibernationfixup as per your advice btw.

Also when macOS shuts down, I use verbose boot. When I shut down or reboot, the monitor doesn’t display the verbose logs. I know it may not be a big deal but I’d like to see them. I mean I still see verbose on startup, but when I shut it down or reboot the verbose doesn’t show (my second monitor will still show a picture until the OS shuts off or reboot).
Should I switch SMBIOS to iMac18,2 or something even tho I just have Intel GPU?
